Dear Perspective Parent,

You can contact us at 228-466-4175 for information on The Early Education Center and programs offered through the EEC along with a registration form.

When you register your child, you will need to bring or complete the following information at the Early Education Center:

	Bring your child’s 121 Immunization Compliance Form from the local health department
	Certified Copy of your child’s birth certificate
	Complete a Parent Handbook Form
	Complete an acknowledgement of summary of Licensing standards and other materials designated by the licensing agency.
	Payment option bank draft or monthly check/money order

The Early Education Center will be available to participate in the The District Child Care Certificate Program.

Information about this program may be obtained by contacting the SMPDD Child Care Department at (601)545-2137 or 1-888-867-6733.

If you plan to participate in this certificate program you must pay the registration fee and one month tuition in full to cover child care until new certificates can be issued in October at that time your co-pay will begin. Co-pays will be required at the first of each month.
